What personal data will I be asked for?

Doctor Zito collects the following personal data, by way of example but not limited to, which will be strictly used for the purposes established in this Privacy Notice: paternal surname, maternal surname, name(s), date of birth, identification, address, sex and signature.

Doctor Zito collects and uses personal data about the holders, for the following main purposes: the provision of medical services, as well as compliance with Laws, Regulations, Circulars, Mexican Official Standards, and any other government provision in charge of Doctor Zito, likewise the data collected may be used for the analysis, validation and/or qualification of legal, operational and/or financial aspects in a specific manner.

Doctor Zito uses personal data for the following secondary purposes, including but not limited to: The marketing, promotion, contracting, placement, certification, and qualification of all types of products and/or services of both Doctor Zito and third parties with whom it has a business relationship.

Doctor Zito may transfer your personal data to the following third parties:

To exercise your ARCO Rights, please send your request to the email address indicated in Section 2 of this Notice. The request must contain the following information and documentation:

1. Name and copy of current official identification of the holder and/or his/her legal representative. In the case of the legal representative, the document proving his/her identity must be included. The documents must be scanned and attached to the email to verify their authenticity.
2. A clear and precise description of the personal data for which you wish to exercise ARCO Rights, as well as the right or rights you wish to exercise, which may be provided in the text of the email or in a scanned attached document. Said document must be duly signed at the end of the email and initialed at the bottom of each page.
3. Expressly indicate your desire to receive a response from Doctor Zito to your request via the email address you provide us.

Once the request to exercise ARCO Rights has been received by Doctor Zito, the respective response will be issued and the request will be processed within the time periods established by law. In the event that the owner is not satisfied with said response, he/she may contact Doctor Zito immediately through the indicated means, remaining at his/her entire disposal to address any concerns. In the event of not responding to Doctor Zito’s response within the indicated period, Doctor Zito will understand in good faith that the owner is satisfied with the response.

If the request to exercise ARCO Rights is about the exercise of the right of access, Doctor Zito will make the information or personal data available to the owner through simple copies and/or electronic documents. Doctor Zito, as the controller, may deny access to the exercise of ARCO Rights, in the cases permitted by the Applicable Provisions on Personal Data, and must therefore inform the owners of the reason for such decision. The denial may be partial, in which case Doctor Zito will carry out the access, rectification, cancellation or opposition in the relevant part.

What are the means that allow personal data to be collected automatically?
